Background:
The authority for requesting this information is the Agricultural Marketing Agreement Act of 1937 [Secs. 1-19, 48 Stat. 31, as amended (7 U.S.C. 601-674)]. Submission of the Tax Identification Number (TIN) is mandatory, and will be used to validate ballots and determine affiliation or identity. If you do not disclose a TIN, the ballot or other information that you provide will not become invalid if there is another means to identify you.
Only authorized employees of the Committee and authorized representatives of USDA, including Agricultural Marketing Service, Fruit and Vegetable Programs’ regional and headquarters staff, will have access to the information that you provide.
All forms in the collection have been approved by the Office of Management and Budget (OMB) in accordance with the Paperwork Reduction Act of 1995, as amended. Any previous editions of the forms have expired and are invalid. Please only use the forms posted above to submit your information to the Committee and to USDA.
