The Plant Variety Protection Office provides the application and other exhibits in Adobe Acrobat Portable Document Format (PDF). Since there are several different crop kinds, the Exhibit C forms are identified by their common names in the drop down list below. A complete application, along with fees, is required in order for your application to be processed.

Exhibit C Forms: These forms were created by the examiner for the applicant to provide a botanical description of the variety for most crops. In order to get a copy of the form specific to your crop, select a form from the drop down menu. The forms are grouped in alphabetical order. In order to get to your form, select the group where the form can be found and hit ‘GO’. For example, if you select Verbena THROUGH Zoysiagrass you will get a drop down list of forms beginning with Verbena and ending with Zoysiagrass. You must hit the ‘GO’ button again to get the drop down list of those forms.

The application (ST 470) can be found under the title, “Application and other pertinent forms.” If you cannot find a form that is specific to your crop, please use the General Crop Form.
